1. Acceptance of Terms

These Terms of Service ("Terms") govern access to and use of MASE EMR and MASE Health OS (together, the "Services"), operated by Serenity Rehabilitation Center, Inc. ("MASE," "we," "us"). By accessing or using the Services, including www.mase-emr.com and https://www.maseneural.io, you agree to these Terms and to our Privacy Policy. If you do not agree, do not use the Services.

2. The Services

MASE EMR / MASE Health OS is an electronic health record, practice management, and related operations platform for healthcare organizations and their authorized workforce. The Services are a tool to support clinical and operational workflows. They do not replace professional judgment. Access to clinical functions is limited to authorized organizations and their credentialed users.

3. Accounts and Acceptable Use

You are responsible for activity under your account and for keeping credentials confidential. You may not share credentials, access data you are not authorized to view, interfere with the Services, or use the Services in violation of law, including HIPAA and 42 CFR Part 2 where those rules apply.

4. HIPAA and Health Information

Covered entities and business associates that use the Services must execute a Business Associate Agreement with us. Protected health information must be handled in accordance with that agreement and applicable federal and state law. Patients should contact their provider for requests about their medical record.

5. Privacy

Our Privacy Policy explains how we collect, use, and share information, including phone numbers and SMS opt-in consent for text messages. Mobile numbers and SMS opt-in consent are never shared or sold to third parties or affiliates for marketing purposes.

SMS / Text Messaging

Program name: MASE Eva Sales recap

Description: MASE Eva Sales recap is a text program operated by Serenity Rehabilitation Center, Inc. through MASE EMR / MASE Health OS. When you call us and verbally agree to receive a text recap, we send one text message summarizing that sales call. Messages are sent from +1 (248) 919-5856. Consent to receive these messages is not a condition of any purchase.

Frequency: Message frequency is one message per call.

Rates: Message and data rates may apply.

Opt out: Reply STOP to opt out. After you opt out, we will not send further program messages unless you opt in again.

Help: Reply HELP for help.

Carriers: Carriers are not liable for delayed or undelivered messages.

6. Limitation of Liability

To the fullest extent permitted by law, Serenity Rehabilitation Center, Inc. and MASE are not liable for any indirect, incidental, special, consequential, or punitive damages arising from use of or inability to use the Services. Carriers are not liable for delayed or undelivered messages.

7. Changes

We may update these Terms from time to time. The "Last updated" date shows when they were last revised. Continued use of the Services after an update means you accept the revised Terms.
